ELSS Home Care is a government-approved provider of Home Care Packages HCP).
Home Care Packages are individual packages of support services that assist older people in the community to remain living in the comfort of their own home as long as possible, also allowing couples to remain together even if their care needs significantly differ. Referrals to this program can come from any source.
To be eligible for a HCP, applicants are first required to have their level of need determined by a local representative of the government’s Aged Care Assessment Team (ACAT). Once the assessment has been completed, an ELSS Home Care Coordinator will visit the client and their representative and together develop a tailored program that establishes the best method of delivering identified care needs, taking into account the cultural values and lifestyle of the Client.
The ELSS Home Care Coordinator works closely at all times with the Client’s doctor and other service providers, as well as family members, friends, neighbours etc. They also become the case manager and the point of contact for this network, which is particularly important in a time of crisis or emergency.
