Southern Plus offers locally based home care services covering the 300km2 coastal area enclosing Bunbury, Margaret River, Busselton and Augusta. Our mobile Support Workers will come to you, no matter where you are within the region. We offer both HACC and Home Care Packages in the region to help you stay at home as long as possible. Home and Community Care (HACC) is a program for lower level home care needs and can give you access to a variety of services. These include assistance with household tasks, nutrition, shopping, independence, centre based day care, nursing care, allied health, respite for carers, podiatry and personal care. Home Care Packages offers different levels of care for people based on what their needs are. A package gives a client an amount of funding to cover their care needs. You can access support for clinical care, social support, transport, domestic assistance, respite for carers, allied health and brokered services. All our clients have access to our specialist dementia and clinical care team if they are needed. Our dedicated team will work with you to arrange any extra services you need to help to achieve your home care goals. We pride ourselves on taking a wellness and enabling approach, helping people to grow in strength, independence and confidence at home. No matter what your home care needs, Southern Plus can help you.
